For item 2 (Authorization Items), please mark ✓ if you agree or ✗ if you do not agree.
The signer’s name must be signed by hand or by electronic signature (typing or pasting screenshots/photos is not accepted).
After uploading, please confirm the file (recommended formats: JPG or PDF) has been successfully uploaded.
The date on the authorization form should be written as Year 114, Month __, Day __.
For other reasons for rejection, please refer to the “Reasons for Rejection” (e.g., uploading incorrect files).
Incomplete submissions will be considered as failing the graduation clearance review.
After completing the form, please photograph the signed authorization (JPG) or save the electronic signature (PDF) and upload it to the School Affairs Information System at:

The online graduation clearance period for the 2nd semester of academic year 113 is from June 2, 2025 to August 7, 2025.
